Photography Notice

Please be aware that throughout the day, photos and videos may be taken of your children during activities or events. These images may be used for promotional purposes, including social media, newsletters, or other communications related to the church. If you do not wish for your child to be photographed or filmed, please let a volunteer or photographer know.

Add your Children

Once you have finished adding in your information, press "Add Child" for each child and enter each childs' information so that we can properly care for them!

Special Needs/Behavioral Issues

Under the medical notes of "Add Child", if your child has any form of Special Needs (i.e. Autism, Asperger Syndrome, Down Syndrome, etc.) or behavioral issues (i.e. anger issues, ADHD, ADD, etc.) that we need to know about to better care for your child, please add that in.

Everything look good?

After you have confirmed your child's information is correct and you have submitted this form, head over to the iPad, type in your phone number, click your child's name and press "Check-In"! A registration tag with their name on it and security tag will print out.

Please place the name tag on your child, and hold on to the security tag to pick up your child from their designated area after service!
